London Gazette, 20 July 1943, Issue 36102, page 3317, under “The Military Medal.”: “No. 4134199 Lance-Corporal Thomas James Lewis, The Cheshire Regiment.” It gives his forenames in full and names his regiment, neither of which this card held.
